Get an Appointment

To get an appointment at the Sleep Disorder Clinic, you must be referred by a doctor (your family doctor or another doctor). Your doctor's request should include the following information:

  • Your name
  • All your phone numbers and an email address
  • Your health insurance number (Régie de l'assurance maladie du Québec) with the expiration date
  • A brief description of your sleep problem
  • The list of medications you are taking or enter "no medication" if you are not taking any
  • The results of any investigations you have previously done at another sleep clinic (i.e., results of a night recording or clinical visits).
  • Please indicate "available at any time" if you want to be reached if an appointment becomes available at the last minute.

Your request will be assessed and prioritized by a doctor at the clinic. Then, a member of our team will contact you to schedule an appointment or to schedule a night of sleep recording, depending on your symptoms.
